Class Registration: 

Gift certificate number (we just began to use #s recently so not all certificates will have a number).
The name of the gift certificate purchaser.
Date your gift certificate was received.
The date of the class you would like to register for. 

Private Lesson or other service:

Gift certificate number (we just began to use #s recently so not all certificates will have a number).
The name of the gift certificate purchaser.
Date your gift certificate was received.
Three dates that you would like Liz to check for a scheduled private lesson or home demonstration.
